The form itself includes, by expanding “Help by common provider”, a cheat sheet with the details of the most common providers — it’s just help information: you still fill in the same generic IMAP/SMTP fields above with those values, there’s no special “Gmail” or “Outlook” checkbox.

Warning — Gmail and Outlook, step by step
For Gmail: go into your Google account → Security → turn on two-step verification if you don’t have it → App passwords → generate a new one and paste it exactly as-is into this form’s “Password” field (not the password you use to sign in to Gmail). For Outlook/Microsoft 365 with two-step verification, the path is the equivalent one in your Microsoft account: generate an app-specific password for this use.
